What is the specialist language examination for physicians?
The specialized language examination is an exam for doctors from abroad. It verifies that their command of medical German is sufficient for working as a doctor in Germany—both when interacting with patients and when communicating professionally with colleagues. The relevant state medical association is responsible for the exam. The examination consists of three parts:
1. Conversation with a patient
The candidate conducts a conversation with a simulated patient based on a sample case—using language that is understandable even to medical laypersons.
2. Writing a report
Afterwards, the conversation is summarized in writing in the form of a medical report (doctor's letter).
3. Case presentation
The case is presented to medical colleagues using professional terminology, followed by a Q&A session.
Those who pass the exam are initially permitted to work in Germany as a resident physician (Assistenzarzt/Assistenzärztin) under supervision.
How is the knowledge test structured?
The knowledge test—also known as the recognition exam—is intended for physicians who completed their medical studies abroad and wish to work in Germany. It verifies that the candidate's medical knowledge is equivalent to the standard of physicians trained in Germany. The responsible authority is the State Chamber of Physicians. The exam consists of two parts:
1. Patient presentation
Examination of and consultation with a standardized patient (an actor) under supervision, followed by a written report covering medical history, diagnosis, potential course of the condition, treatment plan, and a case summary.
2. Oral-practical examination
The focus is on internal medicine and surgery, supplemented by questions on emergency medicine, pharmacology, medical imaging, radiation protection, and the legal foundations of the medical profession.
